changes in ownership or address, or property assessments are not legally handled by the Tax Collector's
Office and should be directed to the
Property Appraiser's Office at (386) 329-0286. This
Name/Address Change Form is to be completed and mailed to the address on the form. Please do not mail this form to the Tax
Collectors Office.
